A Hazard and Operability (or HAZOP) study is defined as a formal systematic examination, by a team under the managment of a trained leader, of the design intentions of a new or existing system or parts of a system, to identify hazards, mal-operation or malfunction of individual entities within the system and the consequences on the system as a whole and on its environment. It typically includes several HAZOP study meetings or workshops. The Keil Centre have experience of the application of HAZOP studies to Human Factors issues associated with the design of equipment and procedures for safety related functions.
